Bug Description

After installing the nightly OpenLP-2.1.1-bzr2467-setup.exe on a clean xp system (registry entries, appdata & program files deleted) and starting it for the "first time" the "Select Translation" dialog box pops up, but it does not have an icon.

I show an OpenLP icon in the top left corner of the popup

Now that I can properly start OpenLP I can see all the title bar icons are missing! Could just be this machine. Somebody has installed a Windows 7 theme!

Could somebody else on windows confirm please?

I've attached a screenshot of the "Select Translation" form when running on Windows 8.1. There is an icon, dunno if it's the right one..?

That's just the default icon. As its the first dialog anybody is going to see I think we should set it to the OpenLP logo. Can you the FTW and others. The icons don't show up on Ubuntu
