db_manage breaking when using mysql

Bug Description
When initializing the database from scratch with the db_manage script, the script cannot initialize the whole database.
I ran "./db_manage upgrade" with the latest barbican code against mysql-5.6
One error is: CommandError: All MySQL CHANGE/MODIFY COLUMN operations require the existing type.
Another is: OperationalError: (_mysql_
This error occurs when the mysql version is > 5.5

Some alembic migration versions are missing some conventions in order to initialize the database when it is mysql.
