Bug Description

In the survey Q&A tab ( /staff/admin/local/action/survey/1 for example), the button color styles have turned into a weirdly separated highlighter effect as a result of the changes to input-group-text styles in Bootstrap 5. Screenshot attached.

While working on the color issues, I discovered that none of the inputs on this screen had labels. I have added them, along with some headings to help navigate the page. Since the colors are also very aggressive, I've changed them to be calmer. The resulting screen is quite different; screenshot attached.
