Add Extension Elimination pass.
Merged from Sourcery G++

It was rejected. Tom is currently working on an updated version that addresses all the issues.
I suggest we *don't* merge the EE pass to gcc 4.6 right away, but we keep this ticket open to remind us to backport the new version when it's ready.
