Update the +subscribe page to handle muted subscriptions properly

Bug Description
Once the work for bug 204980 has landed the +subscribe page will need to be updated to handle muted subscriptions properly. At the moment a user with a muted subscription will be offered the chance to update their subscription, which is a bit confusing since they don't actually know they're subscribed.
Instead, we should show them something like this:
(*) Unmute this bug so that it sends me bug mail
Followed by the usual list of subscription levels.
