Add a toolbar editor dialog

Bug Description

Following the elementary HIG we can add to our apps a dialog to customize the Toolbar. Granite should have a ToolbarEditorDialog to simplify the life of the devs!

If you are interested on the creation of it for granite you can follow and help my work in lp:scratch! Take a look at src/Dialogs/ToolBarEditor.vala (it is not ready jet!)

Marlin had something like that, it wasn't obvious at all.
I don't think it's a good idea tbh, following http://elementaryos.org/docs/human-interface-guidelines/design-philosophy/avoid-configuration; at least the user should not have to switch it (while distributors probably should be able to do that, but they don't need a GUI).

Firefox had it done pretty well.
