application menu is not shown

Bug Description

As shown on the attached screenshot, when I open keepassx it gets the full screen and doesn't show the top bar with the application menu.
It's also not possible to see the left bar.

I'm using Unity 2D on a updated Precise.

This is driving me nuts. I shouldn't have trusted a big part of my memory to an application that I can't hack and fix :)

After upgrading to Ubuntu 12.04, I right-clicked in the toolbar, and inadvertently clicked hide toolbar. Now I cannot get it back. Also there is no place to right-click because the menu bar is also hidden. I tried to uninstall keepassx and reinstall, but the menu bar and toolbar remain hidden. See screenshot. Your help / suggestions would be greatly appreciated - thanks in advance...
