Items out screen should display notice count and date for disabled action triggers

bug 1732275 added a notice count and date of last notice to the patron's Items Out screen. However, those fields will only display values for action triggers that are enabled. It would be better if those fields displayed regardless of whether an action trigger is enabled. If a site chooses to disable an action trigger, staff would still want to know if a notice was sent when it had previously been enabled.
