Bug Description
Observed in 3.3
When a patron completes a self registration form on the OPAC and double clicks the Submit button, multiple entries are generated for the single form submission.

Confirming on 3.4 as well - observed an instance of 124 duplicate rows for a single patron within a 2 minute interval.
