The Provider's Admin UI isn't screenreader friendly - the Search and Reset Form buttons are in the middle of the form, rather than the end of the form. This could be solved by separating them out into their own column, separate from the search input fields.

This fix shifts the Search and Clear Form buttons to the bottom of the form when the form is expanded, which should prevent the issue from occurring.
