The line you refer to in the PDF is not horizontal. The text matrix has
  a small amount of rotation applied to it in the PDF. I suggest filing
a bug report for the extra spaces. As far as I can tell it is not a
cairo bug.

both on OS X 10.5.8:
- reproduced with Inkscape 0.47 r22583 (Cairo 1.8.8)
- not reproduced with Inkscape 0.47+devel r9469 (Cairo 1.8.10)

Can someone else confirm this issue no longer exists with current development builds?

Sorry, I made a mistake and used incorrect export settings when testing with 0.47+devel r9469:
bug still reproduced with Inkscape 0.47+devel r9495 on OS X 10.5.8

I don't know of any extra spaces (should be elaborated more, preferably with an annotated screenshot), but I notice the vertical shift in the letters.
