Bug Description

lp.codehosting.scanner.email.queue_tip_changed_email_jobs decides whether to create a RevisionsAddedJob or a RevisionMailJob based solely on whether it is the initial scan. This means that a very big push can send an awful lot of email.

There should probably be a threshold over which we send a bulk "X revisions added" email.
