Bug Description
I have situation when to identical records for cinder-volume service appears in cinder-manage service list output.
Add unique constrain to services table for host+binary field.

According to 'DB migrations in 9.2' mailing thread, we'll not backport this fix to 9.x and 10.x. We'll prepare a patch for 9.x in case if any customer requests it.
There is a simple workaround for this issue: to remove duplicated entities manually from the DB
