Exclude keystonemiddleware 4.19.0 from test-requirements.txt

keystonemiddleware 4.19.0 has been released with a missing
python-memcached dependency in requirements.txt, breaking code that
depends on it. Exclude that release from our test-requirements.txt to
get the tests going.
