I can reproduce this problem on a Lenovo T61 with a docking station, the monitor layout setting is not used. However, when I call
ApplyConfiguration via dbus, then the correct setting is applied:

dbus-send --type="method_call" --print-reply --session --dest=org.gnome.SettingsDaemon /org/gnome/SettingsDaemon/XRANDR org.gnome.SettingsDaemon.XRANDR.ApplyConfiguration
