Also could you attach the output of "cat /proc/self/mountinfo"

It's possible that this bug is quite different, and that /var/lib/ureadahead/debugfs isn't mounted at all - but an errant entry got added to /etc/mtab by mountall because it was while it was running; and that entry doesn't get removed by ureadahead itself.
