This is only happening after a recent update, which may have included changes to PulseAudio, I'm not sure.
Anyway - the steps to reproduce on my system are:
1. Run pacmd
2. Run list-sinks to see if the C-Media_USB_Audio_Device is selected (has an asterisk next to it)
3. Run set-default-sink if necessary
4. Open Firefox and go to Youtube and play a video
5. Observer the sound is coming from the internal audio not the headset
6. Open Rythmbox and play an MP3, observe the sound coming correctly from the headset
7. Run list-sink-inputs and note that only the Rythmbox sink is listed even though both are playing sound
Expected result:
Sound comes out the default sink at all times
Actual result:
Sound from the browser is not managed by PulseAudio and is just going out the default
