Lock screen incorrectly displays "num lock is on" warning

Bug Description
When the screen is locked an ! icon is displayed that states "num lock is on" even though num lock is off. Turning num lock on or off has no effect, the warning remains. The warning does not appear on the login screen at all (even if num lock is actually on!).
The icon didn't use to be there. I think it turned on only once I turned num lock on and off in a session. Logging out or restarting does not fix the issue.
I'm running Ubuntu 16.04 with Unity 7.4.0 on a Thinkpad T420s.

Sorry, but I don't believe this bug is a duplicate of #1526322, which is about the warning showing up and people thinking its unnecessary.
This is about the warning showing up and never going away, even if num lock is not on.
Though if the solution to #1526322 is to get rid of the warning altogether it doesn't matter much.
