Bug Description

I returned to my desktop this morning, supplied my screen-lock password,
and tried to select a firefox window. The left launcher bar showed almost
useful icon images, but they all had junk over them and were not selectable.
The mouse location and mouse-over tag were only semi-related. See attached
screenshot. UIltimately, I had to restart the UI. Again.

Could you observe closely and if it happens next time you might have some further data about the conditions this bug bug appears, I believe this bug to be somehow related to bug 806248. Also there's very little information about your system in this bug report could you open a terminal and type the following command 'apport-collect 899238' so that this bug report adds a bit information about your system.

For future please report the command 'ubuntu-bug unity' to report a unity bug.

What info do you want about my system? Allowing some random app to collect
and send stuff while opening complete access to every running app just seems wrong.

So, yeah, happened again. Came back from an asleep system and had an empty activity bar. Same transparent color as the background. No icons present, but the bottom third of one at the very top. Unidentifiable. Two or three applications have an active area in the void that cause the app's name to be shown, and will pop-open one of that app's windows. No other behavior.

This latest behavior, but not the first I reported, is similar to Bug #806248 .
