It's always the leader that fails. Adding more units while the one in error is still there is no good (it will deploy but the broken one will still be the one processing relation data). Deleting the one in error state will leave no leader.

With all other charms deployed, removing all keystone units and adding 3 units again, goes into the same state, the leader one enters into error as soon as it finishes installing (the other two go to active/idle).
