As I initially suspected, this seems to be a race condition related to refreshing the rack/region controller. Here's some triage output from jhobbs' system:

It seems that the refresh did not complete properly the first time, which meant that the version information wasn't populated. Upon restarting the rack controller, you can see that the version was eventually populated.

This could also be the possible cause of bug #1722646. We'll continue to triage.
